ACT/Social Work

EHHOP’s Access to Care Team (ACT) is composed of first and second year medical students, master’s students, and Ph.D. students who work closely with social workers to address EHHOP patients’ social needs.   

Here are a few of the types of services ACT offers:

  • Food insecurity and benefits screenings

  • Schedule Mount Sinai REAP Office referrals for legal aid and insurance applications for Charity Care/Emergency Medicaid

  • Help patients find food pantries near their homes (e.g. New York Common Pantry in East Harlem)

  • Schedule ambulatory, mammogram, radiology, and colonoscopy appointments

  • Reconcile outstanding medical bills that should be covered by Charity Care/Emergency Medicaid

  • Provide patients with student case managers who work with patients longitudinally

There is also an on-site social worker during each Saturday clinic who can help patients with many issues including housing, domestic and interpersonal violence, and applying for benefits.
